To request an appointment, please complete the form below. Please allow 72hrs before your 1st selected appointment date. When requesting a date, please take into consideration that you will be requested to complete a patient information sheet, which may take time to complete. Appointments are secured with a £25 deposit which is non-refundable, once the appointment has been confirmed.

Zoom: Tuesday & Wednesday 9am - 7pm

In Person: Thursday 9am - 4pm (new patients latest appointment is 3pm)
